The objective of the study was to evaluate the relationship between the sexual position and severity of penile fracture (PF).
We studied 90 patients with PF. The mechanism of injury and the sexual position was assessed.
We divided our sample by the etiology of the fracture in six groups:
(a) masturbation or penile manipulation;
(b) ‘man-on-top’ position;
(c) ‘doggy style’ position;
(d) ‘woman-on-top’ position;
(d) blunt trauma; and
(e) ‘rolling over’ fracture.
The patient’s age ranged from 18 to 66 years (mean 39 years).
Investigation of the injury mechanism identified sexual trauma as the main etiological factor, involved in 69 cases (76.5%).
The sexual position at the time of injury varied, with
23 cases (25.5%) occurring in the ‘man-on-top’,
37cases (41%) in the ‘doggy style’ and
9 cases (10%) in the ‘woman-on-top’.
It may be regarded as one of the most basic and unexotic sex positions but missionary is actually one of the most dangerous, according to doctors.
In a new study called the 'relationship between sexual position and severity of penile fracture' from the International Journal of Impotence Research, doctors analysed which sex positions were the most dangerous for men.
The study found that 'doggy style' sex is the most dangerous, with 41 per cent of cases of penile fracture occurring as a result of using the position.
Despite being one of the more popular positions, 'doggy style' can lead to the unsuspecting man hitting a woman's pelvic bone, causing it to potentially snap.
Tracey Cox told MailOnline: 'I see why this is the most dangerous of the three – especially if it’s done at the end of the bed.
'All it takes if for him to thrust enthusiastically and her to shift position a little and next thing, he’s fallen out and about to hit the hard end of the bed with force.
'Stop it happening to you by taking firm hold of her hips to make sure she stays in position.
'You’re also safer doing this position on the floor rather than with something hard in front of you.'
She understands why the woman being on top can cause complications, explaining: 'If things get lusty and she leans too far forward or too far back and he has a full erection, it can bend the penis into angles that it isn’t supposed to go.'
Tracey was, however, surprised by missionary being included, saying: 'Missionary isn’t a position I’d classify as remotely dangerous so am perplexed as to how it made it on the list.
'But if he accidentally pulled out and thrust hard against a hard floor, it could cause a fracture.'
They noted that doggy style sex and missionary could cause equally serious penile fracture, while the woman being on top was the least dangerous.
They said: 'We do not observe differences between the severity of the penile fracture between the "doggy style" and "man-on-top", but the "doggy style" had more severity of penile fracture when compared with "woman-on-top".'

I "retired" liao due fucked up company do not employ old man. In my line of work must do Risk Assessment with tons of paper work - auditor always look for documentation. In the end, everyone do paper exercise. No wonder there are continuous work site accidents.

A 47-year-old man fractured his penis during vigorous sex with his partner, a new medical report reveals.
The patient, who remains anonymous, claims he did not notice any symptoms until 12 hours later - despite it normally leaving most in agony.

Doctors at University College London Hospital struggled to examine the member of the patient as it was swollen and 'boggy' to touch.
But they assumed he had a penile fracture, which occurs when the appendage is subject to a sharp, blunt force trauma.
A team, led by Dr Conrad Brice von Stempe, performed an ultrasound to confirm their suspicions, they wrote in BMJ Case Reports.
The test showed a 2x3cm tear in the tubing of the right corpus cavernosum (sponge-like tissue which stores the blood during an erection).
Medics were able to confirm the penile fracture when they spotted a build-up of urine in the penis tissue and a haematoma - blood clot.
The patient was referred to the urology department for surgery, with the position of his injury marked on the skin of his penis.
Surgeons said he recovered within two weeks.
When are penile fractures likely?
Dr Viney said: 'People who try rather unusual positions, such as switching between reverse and normal cowgirl without dismounting, are most at risk.'

A penile fracture occurs when the appendage is subject to a sharp, blunt force trauma, which can occur during vigorous intercourse or masturbation.
Since 1924, 1,600 cases have been recorded worldwide - roughly 16 instances per year, the Telegraph previously reported.
Researchers noted that in 50 per cent of cases, a gruesome cracking sound can be heard. Four in five male victims lost their erection.
Those who have already been traumatised from breaking their penis are often left with erectile dysfunction problems and a lifetime of painful sex.
MailOnline reported on a man from southern China who was left in agony after breaking his penis during sex with his wife in April.
Doctors found that his penis was at an odd angle and was in a swollen shape that made it look like an eggplant.
He was diagnosed with a penile fracture after tearing a sponge-like erectile tissue called corpora cavernosa.
Blood flows into corpora cavernosa that runs along the penis and making it hard during an erection.
The trick to stopping penile injuries is to thrust quite shallowly, according to sex expert Tracey Cox.
Holding your partner close to you using a grinding rather than thrusting motion will also reduce the risk, she told MailOnline.

A woman has told of the terrifying moment she broke her model boyfriend's penis during sex.
Megan Barker, of Guildford, and partner Adam Francis were in the middle of business when a harrowing 'rip' sound interrupted their session.
Megan, 23, said the bedroom was left looking 'like a crime scene' as there was 'blood everywhere.
Her traumatised boyfriend, 24, still suffers flashbacks to this day despite having made a full recovery.
'We're now back to business in the bedroom - we've had a lot of time to make up over the past six months!' Megan told The Sun.
The smitten couple were first introduced to each other through friends in 2013 and instantly fell for one another.
Megan's first thoughts were that model Adam was 'absolutely gorgeous' and that she didn't hesitate to agree to going out for drinks with him.
And it wasn't long before the pair became and item, with an 'extremely healthy' sex life.
But disaster struck in February 2015 as the couple were enjoying a night of passion.

The horrified pair quickly realised something was very wrong when blood started gushing from Adam's penis.

She immediately dialled 999 after entering the blood-spattered bathroom and seeing Adam lying on the floor.
When paramedics arrived Megan said she was too embarrassed to say what had actually happened, instead fibbing that Adam had run into a door.
For the next two months Adam said he was in absolute agony.
'It is indescribable. To add to that, I am not very good with blood so that was what sent me over the edge. I was completely freaking out,' added Adam.
According to osteopath Stephen Makinde, clinical director of the Perfect Balance clinic in London, the number of sex-related injuries is rocketing.
He believes that more and more couples having 'adventurous sex' is to blame.
