There are many reasons why MLs use fake photos. Some are on student passes and work passes so they do not want to be recognised by their friends, colleagues, teachers and employers. In some instances, the employers are aware that these girls are working as MLs or FLs, and they allow the girls to do so, on the condition that they hide their faces.
Then there are girls who are overstayers. These girls hide their faces to reduce the risks of getting caught. They can greatly reduce their risk by constantly changing phone numbers, places of stay, their working names, and even photos (esp changing fake photos).
However, there are others who use fake photos to attract customers. This could be because they are simply unattractive or they advertise themselves concurrently as different persons on different websites (i.e. different names, photos, phone numbers, ages, etc.) so that they can attract customers with different preferences. Xiao Yao and Xiao Dan from SLG use this last strategy - I know because I showed up at their doorsteps on three occasions (and walked away on the 2nd and 3rd occasions on realising that it was still the two of them as I didn't enjoy my first session.)
When booking MLs with masked faces or "too good to be true" photos, I would always ask "照片是本人吗？". There are many girls who would not reply and I would give them a miss. To those who claim that the photos are real, I would state that I've across many fake photos before and would not hesitate to walk away if there are vast differences between the photos and the actual persons, and have no wish to waste each others' time. Those who insist that the photos are themselves almost always turned out to be the case, except for the case of Xiao Yao and Xiao Dan above. In some cases, I've come across girls who, in my opinion, look better than the fake photos they use. One example is this girl, Lily, on $$$$$$.

As for the question of why they use their photos which they had used when they last visited a few years back, the answer is that so that they are instantly recognisable to their old customers. For instance, Vera used her photos from chewyfruit and her loyal customers immediately felt the urge to see her again. Another example was Niu Niu who RTC in July. When she first returned late last year, her business was not too good until it was suggested to her to use a couple of her old photos together with the new ones. This allowed her to re-establish links with her old fan base and business immediately improved. Nevertheless, I agree that those using previous photos should also post their current photos as well, to be fair to their customers.
