On ______(Date) at about ______(Time). I was travelling along ________(Name of road) towards ______ (Where). I immediately swerved to my left side to avoid a collision. As I was moving towards my left to avoid a collision with the said vehicle, I felt a collision impact at the rear right side of my M/Cycle. As a result of the impact, I was thrown off my M/Cycle. Subsequently, I sustained some minor injury and abrasion on my arm/legs/hands (Choose one). Upon checking my M/Cycle, I realized the following damages:-
01) Rear Right Signal light - Broken
02) Front left side mirror - Cracked
03) Scratches on the left side of my bike.

The said vehilce driver's is one:-

NRIC (Driver's NRIC) : __________
Class of D/Licence : 1/2B/2A/2/3B/3A/3/4/5

The said driver did not want to make a Police Report and offered to compensate me for my damages. As I was not seriously injured, I accepted her offer. However, I had tried to contact her without any success.

I would like to add that at the time of accident, the weather was ______(Fine / Raining). the road surface was _______ (Wet/Dry) and the visibilitly was ______ (Good/Bad). The traffic volume was _______(Heavy/Moderate/Light)

I am making this report for record purposes as well as to claim damages against her insurance.

the famous bike at tat time in the group is non other than the fast n super fast..TZR125..no. plate 5076...shall nt disclose tat rider name..last few daes still saw tis bike on ECP....bt of coz different rider liao...last time i myself were riding LC baby..(LC kia)...will the handle bar bent near to the oil tank n the pillion seat oso bent up...ger sit confirm will position herself wif her nehneh poke at you one...last time reali fun...bt tink back..reali waste of time..bt still a memmories...for +++ cock sessions...hehehe..

to have a picture of a lady bending forward on the Patrol tank.
(it will looked like the rider is entering a woman from behind when
he ride) I saw a picture but forgot to take it down and eventaully
forgotten when to find it again. It was not a local picture.
